Use this decision tree if the patient being supported on VA ECMO is under consideration for weaning.

Prerequisites for Coming Off

Prerequisites include:
  • Cardiac pathology is compatible with weaning.
  • The patient has been supported for an appropriate timeframe with ECMO for recovery of the cardiac pathology.
  • Stable hemodynamics with no or low vasoactive agents.
  • Recovering or stable organ function.
  • Adequate anticoagulation to safely perform a VA ECMO wean.
  • Adequate oxygenation and ventilation is anticipated when decannulated.
  • Multi-disciplinary team consensus on prognosis.
Please note: This is just one approach to VA ECMO weaning for adults. There are other approaches you can consider.
