Platelets:

Adult Transfusion Threshold:

Active, clinically relevant bleeding: <50,000 – 100,000/mcL;
No bleeding: <20,000 – 50,000/mcL

Pediatric Transfusion Dose:

10 mL/kg

Caveat:

Exclude heparin induced thrombocytopenia (HIT II)!

Do not transfuse platelets in case of HIT
Stop heparin and start alternative anticoagulation with DTIs
