pH <7.25 and PCO2 > 60 mmHg
Consider:
  • Sedation or neuromuscular blockade
  • Reintubation – rule out obstruction
  • HFOV – low Hz, high amplitude
  • Chest tube(s) if there’s a pneumothorax or effusions
