OI = MAP x FiO2 x 100 / PaO2

Refer to the:
- Sustained elevation and no improvement
- OI > 40 > 4 hours, or > 25 > 48 hours with iNO
- Preductal saturation <90% on FiO2 100%
- Pre/post ductal saturation difference > 20%
- Barotrauma
- Sedation or a neuromuscular blockade
For pneumonia, aspiration syndromes, and/or effusions:
- Surfactant
- High PEEP strategy
- HFOV with increased MAP to optimize lung volume
- Chest tube(s)
- Optimize lung volume
- Inhaled pulmonary vasodilators
- Prostaglandin E1
- Chest tube(s)
- Low PEEP vs HFOV vs HFJV
