Adult Dose^:
Loading Dose: 15 mg/kg IV x 1. You may repeat in 10 min with 10 mg/kg IV x 1 for refractory status epilepticus.
Maintenance Dose: 2 mg/kg/day IV in 2–4 divided doses
Pediatric Dose^:
Loading Dose: 15 to 20 mg/kg IV x 1 (maximum 1,000 mg/dose). You may repeat with an additional 5–10 mg/kg 10 minutes after the loading dose.
Maintenance Dose: 1–2.5 mg/kg/dose every 12 hours
Neonatal Dose^:
Loading Dose: 15 to 20 mg/kg as a single dose or in divided doses. If seizures continue, you may administer repeat doses up to a total of 40 mg/kg.
Maintenance Dose: 3.5 mg/kg/dose IV every 12 hours
