Adult Dose^:
Loading Dose: 3 mg/kg IV x 1 for refractory status epilepticus
Maintenance Dose: 0.75 to 1.5 mg/kg/dose twice daily. May increase up to 3 mg/kg/dose, twice daily.
Pediatric Dose^:
Loading Dose: 2 mg/kg (maximum 200 mg) IV x 1 for refractory status epilepticus
Maintenance Dose: 0.5 to 1.25 mg/kg/dose twice daily. May increase up to 2-2.5 mg/kg/dose, twice daily.
Neonatal Dose^:
Loading Dose: 200 mg IV x1 (typically ~2 mg/kg) for refractory status epilepticus
Maintenance Dose: 50 mg IV twice daily
