Adult Dose:
Initial 300 mg colistin base activity (9 million units) IV, followed by 150 mg colistin base activity (4.5 million units) IV every 12 hours
Pediatric Dose:
Initial 5 mg colistin base activity/kg (150,000 units/kg) IV (maximum dose 300 mg colistin base activity), followed by 2.5 mg colistin base activity/kg/dose IV every 12 hours (75,000 units/kg/dose) (maximum dose 150 mg colistin base activity)
Neonatal Dose:
1.25 to 2.5 mg colistin base activity/kg/dose IV every 12 hours (75,000 units/kg/day-150,000 units/kg/day)
