Adult Dose:
4.5 g IV every 6 hours, prolonged infusion favored after loading dose
Pediatric Dose:
2–9 months of age: piperacillin 80 mg/tazobactam 10 mg per kg IV every 6 hours
> 9 months of age:
- ≤ 40 kg: piperacillin 100 mg/tazobactam 12.5 mg per kg IV every 6 hours
- > 40 kg: 4.5 g IV every 6 hours; maximum dose: 4.5 g/dose, 16 g/day of piperacillin and 2 g/day tazobactam
Neonatal Dose:
100 mg/kg/dose IV
Postmenstrual Age ≤ 29 weeks:
- Postnatal Age ≤ 28 days: every 12 hours
- Postnatal Age > 28 days: every 8 hours
Postmenstrual Age 30 to 36 weeks:
- Postnatal Age ≤ 14 days: every 12 hours
- Postnatal Age > 14 days: every 8 hours
Postmenstrual Age 37 to 44 weeks:
- Postnatal Age ≤ 7 days: every 12 hours
- Postnatal Age > 7 days: every 8 hours
Postmenstrual Age ≥ 45 weeks: every 8 hours
