Imipenem/cilastatin:

ECMO Dose Adjustment:
YES—dose at the higher end of the normal dosing range due to potential for sequestration
Renal Impairment Dose Adjustment*:
YES—a decreased dose may be warranted in some patients
CRRT Dose Adjustment:
YES—the dose may need to be increased relative to the renal impairment dose

Adult Dose:
1 g IV every 6 hours
Pediatric Dose:
1 to 3 months of age (weighing greater than 1,500 g): 25 mg/kg IV every 6 hours
≥ 3 months of age: 15 to 25 mg/kg IV every 6 hours; maximum 1g/dose, 4 g/day
Neonatal Dose:
Bodyweight 2 kg or less:
- ≤ 1 week of age: 20 mg/kg/dose IV every 12 hours
- > 1 week of age: 25 mg/kg/dose IV every 12 hours
Bodyweight 1.5 kg or more:
- > 1 week of age: 25 mg/kg/dose IV every 12 hours
- 1 to 4 weeks of age: 25 mg/kg/dose IV every 8 hours
