Methicillin-resistant Staphylococcus aureus: 6 mg/kg/dose IV every 24 hours
Vancomycin-resistant enterococci: 12 mg/kg/dose IV every 24 hours
Pediatric Dose:
1 to < 7 years of age: 12 mg/kg/dose IV every 24 hours
7 to < 12 years of age: 9 mg/kg/dose IV every 24 hours
12 to < 18 years of age: 7 mg/kg/dose IV every 24 hours
Neonatal Dose:
6 mg/kg/dose IV every 12 hours, consider alternate antimicrobial
