Postmenstrual Age ≤ 32 weeks: 5 mg/kg/dose IV every 8 hours

Postmenstrual Age 32 to 40 weeks: 7 mg/kg/dose IV every 8 hours

Postmenstrual Age > 40 weeks: 9 mg/kg/dose IV every 8 hours
